What I usually do is set nographic=0 and make sure I have X11 forwarding
activated. Alternatively, you can also use SDL or VNC, but I've never
gotten it working: the VGA console is enough for me to make sure it's
working the first time, and afterwards I turn it off and just use ssh.
 
If you don't use SDL or VNC, how do you get to the VGA console?
I can't use ssh, because the domain isn't starting up at all, as indicated 
by the 0.0 in the 'time' field for the HVM domain:

I'm looking for a way to get more debug output to possibly see what the 
problem is.
