what is your base OS and kernel version as well as your xen version.

I haven't seen this before, but for what it's worth, I just finished installing Windows 7 as a domU with the following disk line in my VM config file:

disk = [ 'phy:/dev/sysvg/vm_windo_disk0,xvda,w', 'file:/vm/iso/windows_7_ultimate_64bit.iso,xvdc:cdrom,r' ]
