Are you on xen-unstable? The configuration file format for HVM guests
changed a week or so ago.

I don't think extra does anything for fully virtualised domains.

You might want to try
disk = [ 'phy:/dev/xenfs/slc4root,hda,w',
'file:/root/SLC43_i386_dvd.iso,hdc:cdrom,r' ]
and see if that works any better.
