Above definition is stale and never used nowadays which does 
need cleanup. See set_one_rr where rr7 will be set with vhpt 
table enabled absolutely at the first time when guest tries to 
mov_to_rr. Even VHPT_ENABLED itself is questionable. Does 
anybody ever expect to run a xen/ia64 without vhpt table? I don't 
think so...
