Great, it is fixed.  I now get:

attempting to handle as privop

(XEN) handle_op: can't handle privop at 0xa0000001002d46e0 
(op=0x000000a002102200) slot 0 (type=1), ipsr=0000101208026018
(XEN) priv_emulate: priv_handle_op fails, isr=0000000400000020
done.

but despite those warnings everything works correctly (that code
falls through to delivering IA64_NAT_CONSUMPTION_VECTOR).
