This is in particular helpful for pure PV environments, e.g. the
shim.

1: use CASE_SIMD_PACKED_INT() where possible
2: introduce CASE_SIMD_PACKED_INT_VEX()
3: drop CASE_SIMD_DOUBLE_FP()
4: introduce CASE_SIMD_..._FP_VEX()
5: disable FPU/MMX/SIMD insn emulation when !HVM
