    x86: stub out PV only code in do_debug
    When PV is disabled those symbols won't be available. It is impossible
    for Xen to hit #DB there.

    x86: connect guest creation with CONFIG_PV
    This is a bit more complicated than the HVM case because system
    domains have PV guest type. Leave them like that.
