1: make get_page_from_mfn() return struct page_info *
2: remove _PAGE_PSE check from get_page_from_l2e()
3: improve _put_page_type() readability
4: use switch() in _put_page_type()
5: make _get_page_type() a proper counterpart of _put_page_type() again
