This patch is still broken.  All XenServer HVM guests (both windows and
linux) are dying, with Qemu citing

I/O request not read: 0, ptr: 0, port: 0, data: 0, count: 0, size: 0


The domain still exists, but it looks like it never gets unpaused from
the device model request.
