For those interested, applying the following commit fixes my issue;
found it through a bisection.
83d51ab xen/setup: update VA mapping when releasing memory during setup

I however had to cherry pick lots of previous commits (p2m related) to
be able to apply the patch on top of a 3.4.12 kernel.
It's a shame that it sounds impossible to backport it for the stable
tree since the issue will remain in those stable releases.
