i mean your problem is know, but no known solution exists.
That doesn't mean there is no solution, it just means nobody found one yet :)

It's spoken/written out many places (xen-devel/xen-users) that VGA passthrough 
with nvidia works ok with windows xp, but not with windows versions higher 
than xp. but it works with ati. 

to get to the actual problem:
you could try to also expose the "0xd30...- 0xd307fff" acpi regions.
no idea if that helps in any way i just saw that this is the only region 
you're not mentioning in the patched dsdt.asl

good luck - but please try to accept the fact that nobody _knows_ the solution 
to your problem (well at least nobody i know of).

and about the pdf/wiki - ah... forget it
