Not sure if you have a reason for specifically CentOS, but Oracle Linux is another RHEL “clone” and you can download a free, already created and tested “Oracle Linux template” running a 5.5 PV kernel that should run fine on xen 3.4.3 here:

I am trying to create a PV Centos DomU on CentOS 5.5 (xen 3.4.3). Can anyone provide a pointer how to do the same using an centos.iso file?
