I think it is worth trying with a loop device instead of a file:

- create a loop device on your host
losetup /dev/loop0 /home/yanghy/vm/centos.img

- use 'disk = [ 'phy:/dev/loop0,xvda,w' ]' in your config

If it works it is probably a bug in libxl's blktap support.
