Yes, the option was made per device, configured via sysfs. I think having a global module option in addition is not a bad idea though.

Has the permissive option for pciback been removed? Adding "pciback.permissive" as an option in grub results in the following:
Unknown boot option 'pciback.permissive':ignoring

modinfo pciback (if I compile pciback as a module) shows only verbose_request:int and hide:charp as the only parameters.
