[XENBUS PATCH 0/4] Clean up ballooning code

The series started by noticing that MemoryDecreaseReservation and
MemoryPopulatePhysmap did not have error checking.

Balloon logging was mostly removed to make the xen_platform_log less
chatty. Additionally, the whole fake MDL code path was removed.
