Not necessarily. http://xenbits.xen.org/linux-2.6.18-xen.hg
has pvusb drivers for 2.6.18 kernel.
You could compile those with the el5 kernel-xen. Shouldn't be that
difficult.
Seems like it would be easier to use KVM as passing a USB port seemed
alot easier according to a quick google.
Xen supports qemu USB passthru to Xen HVM guests without any
additional drivers.
It's pretty much the same as what KVM has.

For PV domUs you need to use pvusb.
pvusb requires *drivers* in both the dom0 kernel, and in the domU
kernel!
(in addition to pvusb support in Xen).
