Hi,
While booting DomU. I built another LVM EXT3 partition, booted the same image and that booted fine. Issue seems to be only with using an LVM EXT4 partition.

That shows ext3 module is being used, not ext4.
Try rebuilding initrd using --preload ext4 (or something like that).
Also make sure your fstab has ext4 for "/" instead of auto.
