Absolutely. I searched google and this list. I found many suggestions by
people on how to fix this issue. None helped. This does appear to be a
recurring issue where the root cause has not been identified. Logs have been
no help and strace'ing the process did nothing but give me a headache.

Make sure you're using udev and it is setup properly. What does the
output of "# cat /proc/sys/kernel/hotplug" shows?
