I saw I/D cache synchronization issue again on tiger platform.
In construct_dom0 function, originally we did cache flush just after
loading guest kernel, while subsequent function dom_fw_init also copy
some code segments to domain0 space, so we should do cache flush after
dom_fw_init call. I also fix this kind of issues in other places.
