Bug detailed description:
--------------------------
The issue is after loading myricom driver, then do guest vtd device assignment
will cause xen crash, behavior is like " queue invalidate wait descriptor was
not executed".

reproduce steps changed:
1. Install xen 21237
2. boot to dom0 and load myri10ge driver, then hide one pci device(a Intel NIC)
3. boot one guest with pci device assigned
4. Xen will crash
