I've been able to reproduce this with a very simple domain config:
kernel = "/boot/vmlinuz-2.6-xenU"
memory = 128
name = "bl3-3-vm1"
cpu = -1
disk = [ 'phy:hda5,hda1,w',
         'phy:hda6,hda2,w' ]
dhcp = "dhcp"
root = "/dev/hda1 ro"

It should be noted that it's only the domU that crashes, not dom0.
