While you could probably search the wiki or list archive (I don't have
the exact link, sorry), it's much easier to just use later kernels
(e.g. 2.6.38 works, not sure what the earliest version was), and
simply load xen-platform-pci (which should automatically load
xen-blkfront and xen-netfront after that).
