Summary:

 - Move blktap to drivers/block
   - Takes a .config update setting BLK_DEV_TAP
   - Redo the ABI definitions, in linux/blktap.h

 - Some prerequisites for new commands, such as cache flushing and
   trim support. Plus more device configuration driven from userspace,
   such as setting physical sector sizes.

 - a new CREATE_DEVICE ioctls to flag extensions, like the above
   mentioned ones.

 - Bugfix from Dominic Curran (Fix reference to freed struct request).
