Bug detailed description:
--------------------------
the same vcpu after hot removed cannot be hot added again.


Reproduce steps:
----------------
1.start a HVM
2.xm vcpu-set id cpu_nr
3.echo 1 > /sys/devices/system/cpu/cpu_id/online  
4.remove it: xm vcpu-set id cpu_nr
5.echo 1 > /sys/devices/LNXSYSTM:00/LNXSYBUS:00/LNXCPU:cpu_id/eject
6.then add it again: xm vcpu-set id cpu_nr. it does not work.
