Bug detailed description:

this is a new issue on xen-4.0-rc1, when I hot-add memory, there is no any
response after press the attenton button. the total memory does not increase,
and no useful infomation on both xend.log and xm dmesg. 
if I online physical cpu with memory attached, we can enable the CPU only, the
attached memory can not be initialized in the same time.
I try both ER and SR, the result is the same.
fallback to the old dom0 4d9bf5db06b2b26aceebeb389a4658872ce1c6b8 with
changeset 20715:3c93fffa6502, this issue never happens, so this bug should be a
regression.
